In the last ten years the pediatric neurosurgeon has witnessed a real revolution in the diagnosis and treatment of pediatric hydrocephalus, the most frequently encountered condition in everyday clinical practice. The evolution of MRI and the advent of neuroendoscopic surgery have resuscitated the interest in the classification, etiology and pathophysiology of hydrocephalus. The book offers an updated overview on the recent progress in this field, and a new approach to hydrocephalus: the reader will find in it a modern and new presentation of an old disease, where genetics, endoscopy, cost-effectiveness analyses and many other aspects of the various therapies are extensively discussed. The volume will be useful not only for neurosurgeons, but for all specialists interested in the various aspects of hydrocephalus: pediatricians, radiologists, endocrinologists, pathologists and geneticists.

Advances in Neurosurgery presents the experience and research results of modern neurosurgeons confronted with urgent diagnostic and therapeutic problems. The first topic here is spontaneous intracerebral hematomas, whose localization determines the form of treatment and usually its outcome. Newer therapeutic approaches such as stereotactic removal, endoscopy and fibrinolysis supplement the results of standard operative methods. Secondly, the hydrocephalus malresorptivus is covered, presenting experience with the diagnostic value of intrathecal infusion tests, CT-cisternography, evoked potentials, and MRI CSF-flow studies, as well as aspects of neurosurgical intensive care such as barbiturate treatment, respiratory problems in head injury patients, auto-regulation mechanisms, and hospital infection. The last section is devoted to peripheral nerve surgery, reports on experience with microsurgery and various forms of anastomosis in both traumatic and tumorous nerve lesions.

This book provides the reader with a well-structured, comprehensive approach to hydrocephalus and related syndromes. It also addresses the ethical dilemmas of managing hydrocephalus, of which many neurosurgeons are unaware, and presents cutting-edge research genetic and pathophysiology research on hydrocephalus. The book presents a new classification for the Dandy Walker syndrome, based on a new understanding of different hydrocephalic problems. Further, it puts forward a wholly new theory on the pathophysiology and development of multiloculated hydrocephalus. This book helps the reader to devise a long-term strategic plan to treat patients, based on research of favorable long-term outcomes of hydrocephalus. It uniquely provides evidence-based approaches to managing hydrocephalus, presenting the experience and thoughts of renowned and highly experienced neurosurgeons from the USA, Canada, Japan, Europe and the Middle East.

During the past ten years, technology has been progressing at a rapid pace in the field of medical science, and there have been great improvements in the basic understanding, diagnosis, and treatment of hydrocephalus. The present volume contains the proceedings of the International Symposium on "Hydrocephalus" that was held in Kobe, Japan, from November 12-14, 1990. The purpose of the symposium was to highlight the recent advances in this field and to provide a unique opportunity for exchange of basic and clinical knowledge among experts in this field. During the symposium, 84 papers were presented by 240 contributors from 14 countries. This volume is a compre hensive description of the current knowledge in this area, and it may help to define what is known and to find new strategies to understand the patho genesis, pathophysiology, diagnosis, and treatment of the hydrocephalic process. The editors take great pleasure in expressing their thanks to the contributors for their participation and cooperation, and to Springer Verlag for personal and technical assistance in publishing the proceedings. We sincerely hope that this volume will contribute to improving the treat ment for hydrocephalus. August 1991 The Editors V Contents Preface.......................................................... The Neurosurgeons of Berlin are happy to welcome their colleagues from Germany and from foreign countries to our Annual Meeting, being held in this city once again after many years. The external appearance of the city may have changed in many respects since our winter meeting in 1965. This ist not the case, however, as concerns the outstanding quality of its cultural life. We hope that you take as much profit of it as possible. When choosing the scientific themes of this congress, we were led by the idea that problems should be discussed which are not dealt with at special symposia this year. Thus, the problems related to lumbar disc surgery and its complications were chosen as one main topic. This important chapter of neurosurgery worries (and not seldom depresses) us in our daily work. A joint statistic analysis has been made, for which we are indebted to the colleagues from the following university clinics: Essen, Freiburg, Graz, Hannover, Heidelberg, Kiel, Koln, Mainz, Wiirzburg, Berlin-Charlottenburg, Berlin Steglitz, of the Neurosurgical Department of the BG-U nfallklinik Frankfurt/Main, of the Neurosurgical Department Wagner-J auregg-Krankenhaus Linz, of the Neurosurgical Clinic of the Katharinen-Hospital Stuttgart, and of the Neurosurgical Clinic of the Stadt. Hydrocephalus Smart Shunt System

Hydrocephalus Smart Shunt System

This research aims to design and build a smart shunt system for treating hydrocephalus patients. Unlike current mechanical shunt systems, this new system capable of autonomously managing high-pressure hydrocephalus, continuously collecting Intracranial pressure (ICP) data for diagnosis, and implementing a new control algorithm that use stored ICP readings. This objective would be achieved by combining an on/off valve, an ICP sensor, a microcontroller (includes the control algorithm and the reading ICP program), electronic circuits (booster and ICP internal reader), and a battery. There are multiple steps and tests that have been done to develop the smart shunt system. I chose the piezoelectric bending actuator as the main actuator of the Wisconsin Hydrocephalus Project (WHP) valve. I was responsible of searching and choosing the most suitable actuator for our application. Dr. Christopher Luzzio built an on/off valve incorporating this actuator. I helped him test the valve and these tests were successful. I did an extensive electronic based analysis to find the most suitable driver circuit for the piezoelectric actuator that controls the valve. This electronic analysis resulted in a tradeoff between circuit size and power consumption. I designed the moving window average control algorithm and tested it successfully using a graphical liquid crystal display (GLCD). For the sensor part, I helped the ICP sensor team to investigate the best sensor approach to be used in our closed-loop system. To test the integrity of the system, I did a comprehensive test including the control algorithm with the Graphical liquid crystal display (GLCD), piezoelectric actuator, and driver circuit. This system integrity test showed encouraging results. Also, I did initial estimations and calculation of power consumption for different driver circuits to test the feasibility of long life implantable systems. These power calculations showed that we could use nonrechargeable batteries to run the smart shunt system. In conclusion, the design of a smart shunt system is achievable. The WHP team analysis, tests, and predictions support the feasibility of such system. The short-term goal of the smart shunt system is to maintain the ICP within acceptable limits and lessen hydrocephalus daily symptoms.

Written and edited by leading international authorities in the field, this book provides an in-depth review of knowledge of complications of CSF shunting, with emphasis on prevention, identification, and management. It covers the full range of shunt-related complications and the various associated adverse consequences that remain common despite significant improvements in imaging techniques and therapeutic methods. The chapters are organized into two parts: complications of extrathecal CSF shunt devices and complications of endoscopy. In addition to providing clinicians and investigators with the most pertinent current evidence, the book looks forward to future areas of hydrocephalus research and to innovative therapeutic philosophies. This comprehensive reference book will be an ideal source for neurosurgeons seeking both basic and more sophisticated information and procedures relating to the complications associated with CSF shunting.
