This book provides medical care givers with detailed information on those aspects of adolescence that are of significance in the setting of congenital heart disease (CHD), from anatomic and physiologic changes to behavioral issues. In addition, it explains how care should be organized in order to ensure that the needs of adolescents with CHD are fully met. Both theoretical and practical aspects of the switch from a pediatric to an adult health perspective in CHD patients are outlined in detail, drawing attention to the importance of a structured transition plan and other best practices. As the survival of children with complex CHD improves further, the number of adolescents with CHD will continue to grow. Awareness of the challenges that these patients face is essential if they are to be appropriately prepared to assume adult roles and functioning. Readers will find Congenital Heart Disease and Adolescence to be an excellent source of relevant knowledge and guidance. It has been written for a broad audience, bearing in mind that care in adolescents is an interdisciplinary task involving close collaboration among physicians, specialists, nurses, patients, and relatives.

In May 1990, the First International Symposium on Congenital Heart Disease in Adolescents and Adults was organized in Rotterdam. During these three days' Symposium an overview of the many new problems facing the adult and pediatric cardiologist as well as the cardiac surgeon in this rapidly increasing subgroup of the adolescent and adult cardiac population was given. Chal lenges, controversies, uncertainties and questions were presented and discussed with great enthusiasm. We are greatful that most speakers who participated in this symposium, were willing to write a chapter on their presented topics to be edited in book-form. The final result is here. We hope, it will be of value for both to the adult and pediatric cardiologist and other colleagues, who have to undertake the care of this group of patients, whose management poses such unique problems. Dedicated to the critical management of acutely ill adults with congenital heart disease, this comprehensive book discusses the many challenges faced in the management of these patients, who require intensive inter- and trans-disciplinary care. It provides the first universal review of the practical management of patients with these complex conditions, who survive into adulthood with congenital cardiac malformations and are often affected by other morbidities. For this reason, it reflects a consistent interaction and collaboration between several disciplines: pediatric and adult cardiology, cardiac surgery, pediatric and adult cardiac intensive care, anesthesia, intensive care nursing, nutrition, psychology and many other specialties. It offers concise and pragmatic recommendations, and provides basic and advanced concepts that allow caregivers to anticipate, prevent and effectively treat such pathologies. The book also includes chapters focusing on cardiac-database and risk-factor assessment, organizational and operational topics, advanced mechanical circulatory assistance and pharmacology. Whilst bringing together top international experts who are leading reference programs around the globe, this book is an indispensable teaching tool for clinicians and caregivers involved in the management of critically ill adults with congenital heart disease.

This book presents a systematic overview of the most common cardiovascular defects as they are observed in adults, with a particular focus on the long-term effects and issues that may arise following surgical or interventional repair. In-depth information is provided on the standard of repair or palliation and on the role of different imaging techniques, especially CT and MRI, in the evaluation of these patients. Each chapter includes a detailed imaging protocol and describes the management appropriate to particular conditions and circumstances. Congenital heart disease (CHD) is one of the most common inborn defects. The success in treating CHD in children, even when they have complex cardiovascular defects, is resulting in a steady increase in the number of adults with CHD who need to be followed up clinically and by imaging owing to the potential for late sequelae. In providing expert, up-to-date guidance on selection of imaging modality, image interpretation, and management, this book will be a valuable resource for all radiologists, cardiologists, and pediatricians who are responsible for the care of adult and adolescent patients with CHD.

This book is intended as a comprehensive, practically oriented reference on pulmonary hypertension within the context of adult congenital heart disease (ACHD). After an introductory chapter on pathophysiology, the various types of pulmonary hypertension that may be encountered in ACHD are discussed, highlighting the specifics observed within different patient categories. The diagnostic approach is then addressed in detail, and the last section of the book is devoted to management options, from conservative approaches to interventional treatment and the concept of treat and repair. Management in specific patient subjects, such as pregnant women, Fontan patients, and Down syndrome patients with Eisenmenger syndrome, is fully discussed, and guidance is also provided on palliative care. Pulmonary arterial hypertension related to congenital heart disease (PAH-CHD), despite significant similarities in lung pathohysiology, differs significantly from other types of PAH in terms of mechanism of onset, natural history and management. Mistakes and pitfalls in the management of patients with PAH-CHD are often related to a lack of knowledge or expertise in this condition. Pulmonary Hypertension in Adult Congenital Heart Disease will be a valuable resource and learning tool for all who care for patients with ACHD, both in tertiary practice and general cardiology.

This book describes the risks facing patients with congenital heart disease who wish to become pregnant and the ways in which these patients can best be followed and treated during pregnancy. In addition, the organization of care around the delivery is discussed in detail. The first section provides clear advice on pre-pregnancy risk estimation, counseling of patients, medication use, and inheritance. The specific risks associated with congenital heart defects of differing severity are then explained, with guidance on monitoring and management. The coverage includes simple lesions such as atrial and ventricular septal defects, conditions associated with moderate risk, including tetralogy of Fallot and coarctation, and complex disease such as a Fontan or Mustard repair. The final section is devoted to delivery and considers the mode of delivery, anesthetic use, and postpartum care. Readers will find much information that is underreported in the literature, and the book goes well beyond the European Society of Cardiology guidelines, for example, by considering medical conditions not defined as high risk and addressing the organization of care thoroughly.

Cardiac catheterization has long been a central diagnostic modality in the evaluation of children, adolescents, and adults with congenital heart disease, and in children with acquired heart disease. Over the past two decades, transcatheter interventions have become equally important in the treatment of pediatric and congenital heart disease. Some transcatheter therapies have become established as the standard of care for congenital cardiovascular anomalies previously managed surgically, others serve to supplement surgery in the integrated management of complex lesions, and still others are in their infancy, as pediatric interventional cardiology continues to advance in step with the broader field of pediatric cardiovascular medicine. Since the previous edition of this book, new technologies and techniques have been introduced to the field of pediatric interventional cardiology, much has been learned about established transcatheter therapies, and there has been a substantial increase in the volume of published data on outcomes of transcatheter procedures in congenital and pediatric heart disease. With increasing experience, the role of interventional catheterization has become solidified as an integral component of the comprehensive management of patients with essentially all forms of congenital heart disease. Cardiac catheterization has also become established as a distinct subspecialty within the discipline of pediatric cardiology, and at many centers has become the purview of cardiologists who have received specialized training in interventional catheterization and whose practice is focused on catheterization. This book is targeted to trainees and established interventional cardiologists alike, and is intended as a comprehensive reference for the clinical practice of diagnostic and interventional cardiology in pediatric and congenital heart disease. In the organization of this book, a combination of procedure-based and problem-based approaches has been employed. The structure of the book has been updated to make it more reader friendly, chapters have been added to address new techniques and problems in the field, and greater emphasis has been placed on evaluation of outcomes of transcatheter interventions in congenital heart disease. Section 1 covers the fundamentals of cardiac catheterization, with chapters devoted to pre-procedure patient evaluation, sedation and anesthesia, vascular access and related issues, catheterization equipment ands its use, hemodynamic evaluation, angiography, radiation and radiation safety, and echocardiography in the catheterization lab. Sections 2 through 4 include procedure-based chapters that are organized according to the technical type of intervention. Section 2 is devoted to transcatheter interventions for obstructive lesions, including valvar stenoses and vascular obstructions. Section 3 includes chapters that cover interventions to close various pathologic intracardiac communications and vascular anomalies. Section 4 includes chapters covering interventions performed to create or enlarge intracardiac communications, as well as other interventions. Section 5 focuses on the applications of catheterization and transcatheter interventions in the evaluation and management of specific patient populations. This problem-based or patient-based focus, which is substantially expanded from the previous edition of the book, reflects the expanding role of catheterization in the comprehensive management of pediatric and congenital heart disease, and addresses emerging applications of transcatheter interventions, including management of the patient receiving extracorporeal circulatory support, hybrid transcatheter-surgical interventions, and prenatal cardiac intervention. Section 6 includes two chapters that cover outcome assessment and regulatory issues pertinent to the pediatric cardiology in general and interventional cardiology in particular.
