In vitro fertilization has resulted in an estimated 4000-5000 births in the world. The procedure has been accepted in Europe, America and Australia and several hundred IVF clinics are operating successfully. The newer procedures of GIFf, embryo freezing and donor oocyte IVF have become established and are dealt with in several chapters. GIFf has become the procedure of choice for patients with infertility of unknown origin. Oocyte freezing represents an important new technology which is being developed. The routine IVF procedure has improved slightly; variation in results can be reduced by quality control of laboratory and clinical techniques. Male factor infertility has been dealt with by IVF in mild and moderate cases, but newer techniques will be required to deal with severe problems in the male. Most countries have accepted that the straightforward IVF pro cedure is ethical. Limitations concerning the use of donor oocytes and embryo experimentation exist in some religions and countries; legal control of the new reproductive technologies ranges from the passage of statutes to no control at all. Many countries are still considering the need for legislative control. The text endeavours to indicate new areas of importance and to guide those organizing services as to how to introduce newer technolo gies.

Man is entering a new era as a result of advances in human reproduction. Techniques have been developed to assist in the creation of man-artificial insemination and, now, in vitro fertilization (IVF). Soon, other new methods, based upon current advances of the IVF procedure, will develop to improve the quality of human reproduction. The book describes the conceptual framework and details of technique concerned with in vitro fertilization and embryo transfer (ET). Edwards and Steptoe first described the technique of IVF and ET and the subsequent births of two normal babies. Since then, the success rate of the system has been improved by the use of fertility drugs to provide more oocytes and preincubation to mature the oocyte before fertilization. As a result of the continued research from Melbourne and Cambridge, more than 100 babies have been born. A free interchange of information between the Cambridge and Melbourne groups has led to a predictable success rate of 15%-20% per laparoscopy, and infertility centres all over the world are now copying the techniques. It is an appropriate time to inform doctors and scientists to help them understand the various procedures involved in IVF and ET. While many advances will occur in the future, the establishment of high success rates in several of the critical steps in the procedure-oocyte pick-up rate (90%), fertilization (>90%) and early embryo development (70%-90% )-signifies that some of the new techniques are stabilized sufficiently to warrant transmission of information by text, rather than scientific journal.

"The book is a case study in the regulation of medical innovation and is one of the first to put on record the development of a new medical technology together with an account of the evolution of related regulation and legislation. Focusing primarily on the UK, it documents the development of human in vitro fertilization, the regulation of research and clinical provision by the Voluntary (later Interim) Licensing Authority and the move to legislation and statutory regulation. There is a discussion of the ethical issues involved and an analysis of the success of voluntary regulation. Finally, a summary is provided of the regulation of IVF around the world. The authors have unique access to the records of the Medical Research Council and the VLA."--BOOK JACKET.Title Summary field provided by Blackwell North America, Inc. All Rights Reserved

The Organon Symposia have actually become a tradition, keeping up with exciting developments in reproductive medicine. The purpose of this symposium on "Fertiliza tion of the Human Egg in Vitro" was to bring together the stilllimited number of elinical specialists in the field and to stimulate another group of basic research people to exchange their experiences and knowledge, hopefully promoting elose cooperation between the two groups. It was a kind of scientific "first" that all research teams so far successful in achieving the birth of a healthy baby, fertilized in vitro came together at a workshop conference without a large audience of spectators and reporters, but with a small number of highly critical colleagues from the fields of basic reproductive physiology and comparative developmental biology. This atmosphere allowed for the elose exchange of results, hypotheses, diagnostic and therapeutic procedures, criticism, and respect, and created very productive discussions, all of which furthered the aim of the method: To help more childless couples to have their own babies by the ultima ratio procedure of in vitro fertilization and embryo replacement. The book that has emerged from this symposium will help to disseminate the great amount of information and experience gathered among the scientifically and clinically interested colleagues of many other hospitals and universities who could not be invited to the meeting. At the same time, it will prove that there is much more work to be done in the basic and clinical sciences of human embryology and reproductive biology.

In vitro fertilization (IVF), gamete intra-fallopian transfer (GIFT), and other technologies of assisted conception have been heralded by the medical community and the media as "the answer" for infertile couples. This timely collection of articles discusses medical and social options for couples facing infertility; the effectiveness, safety, costs, and benefits of the new reproductive technologies; and some of the legal and ethical issues surrounding the use of these services. Although now in widespread use, the new reproductive technologies have not yet been fully evaluated. No attempts have been made to determine the need for such services, to compare their effectiveness with other therapies to restore fertility, or to assess the risks associated with such treatments. Since there is no agreed-upon standard definition of infertility, over-diagnosis is often spurred by exaggerated claims of success by medical sources and the media. Over-treatment exposes women to unnecessary risks and has a staggering impact on national health budgets. Bringing together key issues in health policy analysis, this volume argues for a public health approach to infertility, maintaining that far too little attention has been given to the important social, ethical, and legal issues involved. These internationally focused essays address critical issues that have arisen from the proliferation of infertility technologies: Should there be any social criteria for IVF recipients? Does society have a responsibility to deal with the long-term consequences of the technology? What is to be done about the diversion of money, resources, and health professionals' talents away from pressing community health needs into a high technology benefiting only a few? What are the best ways to handle ethically questionable practices such as inducements to women to donate their eggs to IVF clinics and misrepresentation of success rates?
