This second edition of the Handbook of Obstetrics and Gynaecology has become necessary because of the increasing growth in know ledge and changing ideas on Obstetrics and Gynaecology. Never theless, we have attempted to maintain the same basic approach and concepts to student learning as we evinced in the first edition. Any changes to the text are updating of existing practices and knowledge or correction of errors in the original text. The Handbook is still a guide to achieving mastery of the undergraduate curriculum, but over the past five years we have realised the book is also widely used by general practitioners and nurses as well as undergraduate students. It is tempting to enlarge the handbook to encompass all the needs of diverse groups of health personnel, but we have resisted this desire. Instead, the Handbook of Obstetrics and Gynaecology can be regarded as a true guide to aid students in the application of knowledge to the clinical setting. In conjunction with one of the major recom mended text books, this book provides a useful aid to students and general practitioners. I hope all who read it continue to find it useful. BARRY G. WREN 1984 Chapter 1 History Taking and Physical Examination BARRY G. WREN GENERAL INSTRUCTIONAL OBJECTIVE The students will develop competence in taking a history and perform ing a physical examination so that obstetric or gynaecological abnormali ties can be recognized and appropriate management initiated.

An updated edition for our popular Clinical Cases collection, this book is aimed at medical students following the core curriculum in Australia and New Zealand; and the United Kingdom, junior doctors at resident or house officer level, and general practitioners. This handy, lab coat pocket sized book broadly covers the whole syllabus for the medical student, and the whole range of obstetric and gynaecological problems likely to come into the path of the busy junior hospital doctor or general practitioner, but it is not a textbook providing simply a list of differential diagnoses and treatments. The approach has been developed from the experienced authors teaching of problem-based learning scenarios PBLs to clinical students. PBLs introduce students to typical clinical situations and then encourage those students to think about how they would take appropriate histories, examine, investigate and finally treat their patients. In this book, by using case histories that are more developed and complex, we aim to show readers in greater detail how women may present with particular conditions, and demonstrate what should take place in the way of consultation, investigations and treatment. However, by questioning our readers as we go along we also hope to encourage them to think about why they would choose a certain clinical course of action and to base their decisions firmly on current scientific evidence. In addition to the purely clinical aspects of the cases, the emotional, social and psychological aspects of the care of each woman is described. Each of the 50 case histories commences with a straightforward description following a woman through the clinical presentation of a particular condition. Along the way, important points in clinical examination and diagnosis, complications, investigations and management are incorporated into the text as a conversation with the reader, and essential points are highlighted in boxes. Being an Australian author team, tips are included for those junior doctors faced with obstetric or gynaecological problems in a smaller metropolitan or rural setting—facilities for care and the advice of senior practitioners may be more limited in such surroundings and different care paths may be more appropriate, especially in remote parts of Australia. This new fourth edition of the Oxford Handbook of Psychiatry is the essential, evidence-based companion to all aspects of psychiatry, from diagnosis and conducting a clinical interview to management by subspecialty. Fully updated to reflect changes to the legislature and classification of psychiatric disorders, and with coverage of the anticipated ICD-11 coding, this Handbook provides the latest advances in both clinical practice and management today. As in previous editions, the Handbook is indexed alphabetically by ICD-10 and DSM-5 codes, as well as a list of acute presentations for quick access in emergency situations. The practical layout helps the reader in making clinical diagnosis, and suggested differential diagnosis makes this title an invaluable guide to provide reassurance to health professionals when dealing with psychiatric issues. With a new chapter on Neuropsychiatry and a re-written section on gender dysphoria to reflect the biological and cultural developments in understanding and research since the previous edition, and filled with clinical observations, guidance, and commentary that reflects the authors' practical experiences of working in psychiatry, this Handbook is the indispensable guide for all trainee and practising psychiatrists.
